Signed Code of Conduct and Emergency Release forms. (Note: you must still mail these forms in to the address provided.)

A copy of your most recent middle school report card or high school transcript.

A 200-300 word typed essay stating why you would like to attend SI and what you expect to gain from the experience.

$175 workshop fee* (make check payable to: University of Colorado)
